Documents Required for a Live Scan

Live Scan Form

  • Acquire a California Department of Justice Live Scan form (BCIA 8016) obtained by the organization requesting your Live Scan. Your organization will need to tell you the specific details to put in the "Applicant Submission" section of the form


Valid Government Issued Photo ID

  • Make sure you have a non-expired valid government issued photo ID. Some examples are a California Drivers License or ID. An out of state drivers license or ID, Passport, Military ID, Immigration Card, Green Card, etc


Form of Payment

  • Contact your organization and find out if they will be paying for your Live Scan fees, or if they are having you personally pay


  • Pricing fees vary on the each Live Scan, reach out to us for specifics on fees


  • Cards only, no cash
